Game Request Form

Submits a request for the purchase of a game for use in a class or for research. Please submit a consultation request if you would like help finding a game or if you have any questions.

Game Request Form

  • Please provide your email address.
  • Please provide your class name, ID, and brief description.
  • Please provide the number of students in your class.
  • If you plan on a whole class playing simultaneously, five copies are required. Otherwise, only one is required.
  • Please provide a brief description of how you plan to use the game.
  • Please provide the game title.
  • Please provide the console the game requires.
  • Please provide the game's price.
